I use CF7 for all the forms, from email contact to surveys for all my client's websites. This form builder is simple to use and very reliable. The developers do a good job of keeping the plugin up to date and compatible with the latest versions of WordPress. I highly recommend this plugin when you need to include forms on your website.

I've had nothing but good experiences with this plugin, and have used it on many of my sites. The only thing that could be improved is the level of documentation.
